Community Tracking Survey - Household and Physician Survey Information

The Community Tracking Survey (CTS) is conducted by the Center for Studying Health System Change (HSC) and funded by the Robert Wood Johnson Foundation. SHADAC hosted a conference call in March 2004 to provide information to state on the Household and Physician Surveys.  The first attachment provides information on the CTS, and the second attachment presents a summary of SHADAC's conference call held on March 10, 2004.
