This brief from the research team led by SHARE grantee Andrew Coburn, Ph.D., Professor and Chair of Health Policy and Management at the Muskie School of Public Service at the University of Southern Maine, examines how and to what extent states allow health plans to vary premiums by geographic rating area, using insurance data from selected states to assess the direction and magnitude of variations in rural and urban geographic rating factors.  The authors also discuss strategies that federal and state policymakers might use to help ensure that premium variations based on geography are justified.

The Iowa Department of Revenue (IDR) and the Iowa Department of Human Services (DHS) collaborated in 2008 in order to collect necessary information via tax forms to decide which Iowa children are uninsured yet meet the requirements to qualify for Iowa’s CHIP or Medicaid. Iowa used tax forms to determine which children had family incomes that met the Medicaid and CHIP requirements but were not enrolled in these programs. After identifying eligible kids who are not yet enrolled, DHS sends pamphlets and applications to the identified families. The families then become aware of the programs and their children’s eligibility.

In this brief we describe an important decision that should be considered in analyses of health insurance coverage using survey data:  Defining the "family unit" for examining insurance coverage, often called the health insurance unit.  We propose a general definition of the health insurance unit and provide Stata and SAS code to facilitate implementing that definition.

In this brief, SHARE grantee Deborah Chollet and her co-authors Allison Barrett and Amy Lischko analyze risk selection over time in Massachusetts' CommonWealth Care program, which serves low-income adults ineligible for Medicaid, much like a Basic Health Plan under the ACA.  Based on their findings, the authors offer lessons for other states considering whether to form a Basic Health Plan, looking at questions of program design, impacts on the commercial market, and financing and operations.
