Coordinated State Coverage Survey

2005 The Coordinated State Coverage Survey (CSCS) is a household telephone survey developed by SHADAC staff for estimation of health insurance coverage at the state level.
- The first attachment describes the core survey instrument that includes questions about health insurance status and basic demographic information.
- The second attachment provides a flowchart of the CSCS content.
- The third and fourth attachments are a sample questionnaire from the 2007 Minnesota Health Access Survey (Word or PDF versions).
