HRSA Benchmarks

SHAP Benchmark forms have been created and tailored for each state, based on SHAP initiatives outlined in the state’s grant application and feedback received from grantees. Forms will be sent out at six-month intervals to contacts at each state. To request the form for your state, please contact Elizabeth Lukanen at elukanen@umn.edu.

The benchmark forms contain questions related to cost and impact for each SHAP initiative. Questions vary slightly for initiatives that provide health care coverage expansions. The data collected on these forms will be used to demonstrate progress to HRSA. Should the form require amendments or changes please use the contacts above to comment and provide feedback.
